Important Nomination Notes!

Don't want your nomination form to be rejected because you filled out incorrectly or incompletely? PLEASE read the guidelines carefully and include ALL information required, or your nominations may be rejected!

In particular:

For categories 1 and 2, you should include not only the name of the actor/actress and the series, but also the name of the episode that you want to nominate them for. To help you do this, our Eligibility List page includes links to the imdb entries for each item, which includes episode guides. Just click on the title or the image of the series you are interested in nominating to find this info.
For category 7, you must include the exact technical aspect of the show/movie you are nominating. You couldn't simply, for example, nominate the film "Spiderman 14", you would have to be more specific, such as "Costume Design in Spiderman 14".
If you nominate a script from a TV series in category 8, you must again let us know what episode you are nominating. Nominations that just list the name of a series will be rejected.
In all categories, please be sure that the episode or film you are nominating was released in Canada in 2012, not 2011 or 2013. Check episode guides or the imdb if you are unsure.
As noted in the nomination guidelines, any ballots that do not have a full name and postal address on them will be rejected, period.

Also, please note that you do not have to make nominations in every category. If you only submit nominations for some of the categories and leave the rest blank, that is perfectly acceptable and your nominations will indeed be accepted.

For your assistance, we have put a sample ballot online to help guide you as you fill out your ballot. Note that the names, series, and films listed on this sample ballot are, of course, purely fictional - the ballot is simply to help demonstrate what information is required in each part of the form.

Nomination Guidelines

Anybody may submit nominations for the nine award categories.

There is no limit to the number of nominations you may make.

Performers may be nominated multiple times if they performed several eligible performances in 2012, but each distinct performance or work may be nominated only once per category.

The awards committee will review all nominations received and develop the final voting ballot, based on nomination frequency and eligibility. The committee reserves the right to reject, group, or reclassify any received nominations based on our eligibility guidelines. For example, nominations for the "Best Television Series" category that were for a work that were properly categoried as being a "mini-series" might be moved by the committee from category 3 to category 6.

Nomination forms received without names and addresses will NOT be processed. Names and addresses will be used for various tracking purposes (e.g. to track geographical statistics on where nominations are coming from).

Forms must be received by us by March 1st, 2013.

The fastest way to nominate: Use our online nomination form. Your choices will sent to us automatically. Note: You must have JavaScript and cookies enabled in your browser to use this form.
Download the PDF version of our nomination form. This is a fillable PDF that you can either fill in electronically, for printing or emailing, OR you can just print it off blank, fill it in by hand, and send to us by postal mail.
Download the Microsoft Word version of our nomination form that can be filled in via your word processor and sent to us by email.
Duplicate the information from the nomination form into another format and sent it to us by email.
